How to Enrol – Child's Play Before & After School Care

HOW TO ENROLL

Please fill out the Enrolment and Debit Success Form ( Direct Debit) below. Make sure you complete all sections on the enrolment form to ensure you receive the government subsidy. Print, sign and return both forms by either:

Via email: cpbasc@gmail.com (preferable) or n person at AIA’s main office

Email or Give to the Office.

Please note the Family Assistance Office has established guidelines for Priority of Access that we must follow when filling vacancies:

Priority 1 A child at risk of serious abuse or neglect.
Priority 2 Sole parent or both parents working, seeking work, or studying.
Priority 3 Parents/Carers not working.
